 The forms used for financial and accounting such as licenses, receipts and so forth only needs the approval of Treasury, if the format of the forms are to be amended, modified or adjusted.

This requirement is set out in Treasury Circular No. 9/79 and Treasury Instructions 292.

Applications to amend or modify the format of the forms should be submitted to BPKS to ensure that all regulations related to Financial Management such as audit trail and accounting matters, and security features  of those forms were taken into account and complied with.
